A mental health professional is a health care practitioner or social and human services provider who offers services for the purpose of improving an individual's mental health or to treat mental disorders. This broad category was developed as a name for community personnel who worked in the new community mental health agencies begun in the 1970s to assist individuals moving from state hospitals, to prevent admissions, and to provide support in homes, jobs, education, and community. These individuals (i.e., state office personnel, private sector personnel, and non-profit, now voluntary sector personnel) were the forefront brigade to develop the community programs, which today may be referred to by names such as supported housing, psychiatric rehabilitation, supported or transitional employment, sheltered workshops, supported education, daily living skills, affirmative industries, dual diagnosis treatment,[1] individual and family psychoeducation, adult day care, foster care, family services and mental health counseling.

This membership is only for 1 individual between the ages of 18-59. This membership can be based on a month to month rate or a annual rate. If member wishes to cancel they must give a 30 day written & verbal notice.

Couple / 2 Individuals

This membership is for 2 Individuals. Both Individuals must live at the same address and show proof. This membership can be based on a month to month rate or a annual rate. If member wishes to cancel they must give a 30 day written & verbal notice.

Family / 3-8 Individuals

This membership is for 3 or more individuals. Can have up to 8 individuals on a family account. If you need to add more than 8 individuals to your account an additional fee will be charged. Individuals over the age of 18 must show proof of address; such as Governmental ID & Utility Bills. Packaging slips from distributors will not be accepted. This membership can be based on a month to month rate or a annual rate. If member wishes to cancel they must give a 30 day written & verbal notice.

Student

This Membership is only for Students who are attending High School or attending College. This membership is also for individuals 13 and Older. This is based on a month to month rate. If member wishes to cancel they must give a 30 day written & verbal notice. Students under the age of 18 must have parent/guardian signed permission. Must show proof of enrollment of school/college. Student id or Report card is accepted.

Military / Municipal (Individual, Couple, Family)

This membership is at a discounted rate. This is contracted with a locked in 12 month commitment. This can be based on a month to month rate or a annual rate. If member wishes to cancel they must wait until there contract has been fulfilled; along with a 30 day written & verbal notice. 

Corporate (Individual, Couple, Family)

This membership is at a discounted rate. This is contracted with a locked in 12 month commitment. This is can be based on a month to month rate or a annual rate. If member wishes to cancel they must wait until there contract has been fulfilled; along with a 30 day written & verbal notice.

Senior (Individual, Couple)

This membership is only for individuals who are the age of 60  or older. This membership is at a discounted rate. This membership can be based on a month to month rate or a annual rate. If member wishes to cancel they must give a 30 day written & verbal notice.

Insurance

Did you know that some Insurance Plans will pay for a gym/fitness membership. Check with your insurance provider and see if your insurance has a gym/fitness plan. Better yet, give us a call and we can check for you. please note that not all insurance companies have a gym/fitness membership plan. 574-936-2333

Auto-renewal Monthly

Pay Per Month! Membership auto-renews the first of each month. Can cancel after given verbal or written 30 day notice.

Auto-renewal Annual

Pay in Full Per Year! Membership auto-renews starting date of each year.  Client can cancel after contract has been fulfilled and a verbal & written notice is given.

Auto-renewal Monthly Special (12 Month Commitment)

Pay per month at discounted price locked in for 12 months. Membership auto-renews the first of each month. Client can cancel anytime after 12month commitment is fulfilled. Benefit with this membership is after the 12 month commitment is up the price is locked in until client cancels membership.

Auto-renewal Annual Special (Yearly Commitment)

Pay in Full Per Year at discounted price locked in. Membership auto-renews starting date of each year. Client can cancel after contract has been fulfilled and a verbal & written notice is given.

CANCELLATION POLICY

30 day notice & Signed Cancellation Form

All Members must give a 30 day notice to cancel their membership; both verbally & written. All Members can cancel after contract has been fulfilled.   Check with Membership to see if you have any stipulations on your account.

Medical Cancellation

Fitness Forum Sports & Wellness will allow Members to cancel on Medical Cancellation. Clients must provide a Written Letter from their doctor/physician explaining why they can no longer workout in order for them to use a Medical Cancellation.
